2009 Retreat Registration

We are excited your campus will be joining us this year for the Annual OSA Retreat August 28-30, 2009!!
Please fill out the registration form below and be sure to check with your delegation members about any dietary or other needs they might have. If anyone in your delegation needs ADA or other services please contact us by August 1st at (503)286-0477 so we can make sure to provide all accommodations necessary.

Registration includes lunch. Cost is $25 per-person and the final deadline for registration is Friday August 15th.

1 I will mail a check ASAP payable to the Oregon Student Foundation and will send it to: 635 NE Dekum Street, Portland, OR 97211
2 I will bring a check to the event or pay by cash.
3 Please send an invoice to me at the address above. I will mail or bring a check afterward.

Please note
You are responsible for paying for the full number of individuals registered with your delegation even if less than that number show up. We make purchases based on the number of people pre-registered for the training. You can exchange individuals on this list with others even up to the day of the event without notifying staff so long as the total number of attendees has not increased or decreased.
